EOLC Hub (SCFT)

The End of Life Care hub is called ECHO (End of Life Care Hub for Coastal West Sussex) and is commissioned by Coastal West Sussex CCG and delivered by SCFT in partnership with St Barnabas House, St Wilfird’s Hospice and the Midhurst MacMilan service.

ECHO is a telephone service where you can access support and advice for patients who are approaching and entering the end phase of their lives or have received a diagnosis of a life limiting illness. ECHO nurses are available to take your call 24/7, 365 days of the year. More detail can be found at the ECHO website.

Available support includes:

  • Co-ordinated advice from specialist nurses and/or doctors
  • Advice on a change of condition
  • Access to care plans containing the patient’s wishes at End of Life
  • Advice about accessing equipment or practical care support
  • Access to bereavement support

Referral pathway

Referrals can be made by any healthcare professional, carer, family member or patient.

Speech and Language Therapy (SCFT)

Support available includes:

  • Communication or eating and swallowing problems following neurological impairments and degenerative conditions, including stroke, head injury, Parkinson's disease and dementia
  • Head, neck or throat cancer
  • Voice problems
  • Stammering

SALT service

Tissue Viability (SCFT)

The service will be provided in care homes where patients are unable to attend clinics. Support includes:

  • Advice and help with all aspects of tissue viability
  • Advice about pressure ulcer grading and skin care
  • Education for health care professionals

  • Care home with nursing staff via post, phone or fax to details below
  • Any other Health Care Professional via post, phone or fax to details below (Care homes without nursing should go via another HCP such as the GP)
